Please explain why you are applying for the position(s):

I believe this employment opportunity would be better suited to my needs than my current position. I am currently employed as Charge Nurse at St. Christina Psychiatric Rehabilitation Center, but I am finding that the location is not agreeing with me. I believe I would feel much more at home living and working in your state-of-the-art facility.

Why do you feel you would be the best candidate for the position(s)?:

Besides having been among the top of my graduating class in university, I'm a very gentle, compassionate individual, and I pride myself on indiscriminately treating individuals with the utmost of care. I'm a great listener and a fantastic caregiver. I'm flexible with my schedule, a quick learner, courteous, conscientious, punctual, agreeable and fair. I am willing to do whatever the job requires of me, and I'll do it with a smile on my face, which, if you ask anyone who knows me well, is not an exaggeration in the least. Taking care of people is what I live for, and you will never see me hesitate to lend someone a hand. I have plenty of experience working with patients with various ailments and afflictions, from the elderly to the mentally ill.

Yes. I am a Registered Nurse and have a Bachelor of Science in Nursing from Tulane University's School of Medicine.

What other qualifications or skills do you possess which you feel will aid your performance in the chosen position(s)?:

I interned at Tulane Medical Center in New Orleans, Louisiana as I was earning my Bachelor's degree at the university and working at Shady Grove, a nearby nursing home. From doing this, I learned that I possess seemingly endless reserves of energy and excellent time management skills. I attended class in the mornings, went to my internship in the afternoons, and worked for a few hours in the evenings at Shady Grove. I came very close to burning myself out, but the important thing is that I was successful and learned the value of hard work and compassionate care. I can cope with a hectic schedule and I like being busy, which is one of the many reasons why I am well-suited for the medical field.

Aside from my work in the nursing program at Tulane, I have worked as a Caregiver at the Shady Grove Nursing Home in New Orleans, as a Nurse at the Hadley Institute for Troubled Youth in Phoenicia, NY, and as Charge Nurse at St. Christina Psychiatric Rehabilitation Center, Green Ridge, OR.

As you can see, I have plenty of experience and expertise in the medical field. Although my I have not been in my most recent position as Charge Nurse at St. Christina's for very long, I feel it has given me an idea of what to expect at your facility. During the time I worked in that position, I rose to the challenge of the numerous responsibilities I was given, impressing my superiors. My colleagues couldn't say a bad thing about my work ethic if you asked. I am fully qualified for this position and eager to get started, and I am sure that every bit of my past experience will come into play and help me along this new career path, making me even better at what I do.

You're in a desert, walking along. You look down and see a tortoise. You reach down and flip the tortoise over on its back. The tortoise lays on its back, trying to right itself, but it can't. Not without your help. But you're not helping. Why?:

I'm curious to see if it can do it on its own. It might look impossible, but I have faith that it can.

You find a glass. The total volume capacity of the glass is taken up half by water and half by air. Is the glass half empty, or half full?:

Half full.

What methods would you employ in order to relax during times of stress, and how effective do you think these have been in the past?:

I would go out for a drink or surround myself with friends in some capacity. I thrive on the company of other people. I think these methods have proven very effective in the past. They keep me calm and centered, and I heartily believe in the principle of balancing work with play time. I also like to dance and occasionally practice yoga. I find physical activity is a very healthy pursuit and provides a great outlet for stress. I'm not particularly concerned with keeping myself in shape, rather I exercise to keep my mind clear and uncluttered. It works wonders, and staying slim is a bonus.

Please list three traits about yourself that you consider negative, and three traits about yourself that you consider positive. Explain these choices if necessary:

One trait that might be considered negative is my tendency to be too bold, forward, or overly flirtatious. It's in my nature, but I know that it makes some people uncomfortable and that it is not appropriate in a work setting. I also tend to be too trustworthy for my own good sometimes. Another negative thing about me is that I don't like to be alone. I find it difficult to keep still and find solace in my own company.

One of my positive traits is that I'm very outgoing and easy to talk to. People feel comfortable coming to me with any problems they might have, and it gives me a sense of pride to be able to assist them in any way I can. I'm a good listener and, for the most part, a rational and objective person. I'm also a very gentle and kind individual, and I treat everyone I meet as though they are already a friend.

Describe one event in your life that you would change if you could, and why:

My father passed away last month, and I would undoubtedly change that if I could. He was sick with colon cancer for many years, and though he was receiving treatment for it, the illness grew progressively worse over the course of the last year. I took the job at the Hadley Institute to be closer to home in the hopes that I could help care for him, but I didn't get the chance to spend much time with him. I would have changed that if I'd known we would have been relocating to Oregon before I got the chance to say goodbye. I have always had a good relationship with my parents and miss my father more than words can say. I wish he did not have to suffer as he did, and that there was something more I could have done to ease his pain.

If a superior gave you a task to complete, to which you were wholly, morally opposed, what would you do?:

If I felt morally opposed to anything I might be asked to do in this position, I would not have applied for it. However, if I was asked to do something I was vehemently against, I would voice my concerns and try to reach some sort of compromise with my superior.

If you caught a friend and co-worker stealing from your place of employment, breaking rules, and/or endangering the safety of others, what would you do?:

I would pull this individual aside for a conversation and hope that I can make them see the error of their ways before their damaging behaviors escalate. If I found out that they were continuing this behavior after the fact, I would feel that I had no choice but report them to their immediate superior. If I was their supervisor, I would suspend them from their duties for a short period of time before giving them a second chance to redeem themselves.

◦ ENVIRONMENTAL SCREENING ◦

Where did you grow up, and what was it like there?:

I grew up in a small town in New York called Middleburgh, only an hour away from Phoenicia, where the former Hadley facility was located. It was a nice, quiet place, offering all the comforts of country life, and situated only three hours away from New York City. It was close enough to get away for a day or two, but far enough away that we could still enjoy a calm, suburban existence away from the violence, bright lights, and noise of the city. It was a little too quiet for me, but overall, I'd say it's a good place to raise a family. My parents chose well.

What are/were your social interactions like? Do/did you have many friends and/or relationships?:

I never had any shortage of friends. I came out as gay in high school, and while I was worried that I would be viewed differently for it, I was instead accepted with open arms by my friends and family. That's not to say I didn't get a few funny looks from people around town, but for the most part, if they didn't agree with my lifestyle, they left me alone. I guess you could say I've had something of a charmed life. My parents weren't wealthy, but they made sure that I never went without. I had a pleasant upbringing as an only child.

What prompted you to pursue the career path you are now following?:

I didn't decide what I wanted to do with my life until late in my high school career. I spent a lot of time hopping back and forth between Middleburgh and NYC with my friends during those years, but I don't consider any of that time wasted because I think it helped me reach the conclusion that nursing was the career path I wanted to pursue. I've always been a very caring individual, and I took care of a lot of sick friends after long nights spent in the clubs, so I decided that maybe I should give nursing a shot. Taking care of people has always been second nature to me. I'd had a lifelong dream to someday live in New Orleans, so I took a chance and applied to a few schools there, as well as a few in New York. When I was accepted to Tulane, it was a dream come true, and life has only continued to get better and more rewarding each day since. I couldn't be sure that I was making the right decision at the time, but now I know that I chose well.

Do/Did you ever abuse controlled substances, including but not limited to alcohol, prescription medications, street drugs and tobacco?:

I'm a social smoker. I crave cigarettes the most when I'm drinking and in good company, but otherwise I don't make much of a habit of it. Likewise, I like to drink in casual and/or social situations, but I don't abuse alcohol. I have tried marijuana and ecstasy in the past, but only once or twice. It never struck me as wise to dabble with recreational drugs while I was going for my nursing degree. I am not addicted to any controlled substance.

Have you ever been convicted of a crime? If yes, please list conviction(s) and served sentence(s) below:

I haven't. I make no secret of loving to party because I haven't hurt anyone with it. My record is clean.
